Family Planning Benefit Program

The Family Planning Benefit Program (FPBP) is a public health insurance program for New Yorkers who need family planning services, but may not be able to afford them. It is intended to increase access to confidential family planning services and to enable teens, women and men of childbearing age to prevent and/or reduce the incidence of unintentional pregnancies.

Planning to apply for the FPBP?

Here are some things you may want to bring to your appointment (if you don’t have them all, come see us anyway):

  • Photo ID (such as a school ID card, library card, etc.)

  • Social security number (you don’t need the card, you just need to know the number)

  • Proof of citizenship or immigration status (US-born citizens can use social security number for this. Naturalized citizens can provide a Naturalization Certificate or US Passport.)

  • Proof of age (such as a driver’s license/learner’s permit, Passport or birth certificate)

  • Proof of earned income, if you are working

  • Proof of unearned income

  • Proof of where you live (such as a piece of mail with your name and address on it)

  • Any valid health insurance benefit card, if you have one

  • An alternative address where you can receive confidential information if you do not want to use your home address
